Is this form for you?

One question decides: who delivers your practice's figures?

This form

Your software provider or group delivers the figures

  • You only answer questions about your practice: structure, staff, equipment and billing. No figures per doctor.
  • Takes 10 to 15 minutes.
  • Have ready: the identifier from your software provider (if you received one) and your practice's key facts.

Excel workbook

You deliver the figures yourself

  • No export through your software provider or group.
  • You enter monthly values from January to June 2026 for each doctor in the workbook.
  • Takes 30 to 45 minutes with a software extract, about an hour when estimating.

What this is about

TARDOC replaced TARMED on 1 January 2026. For tariff version 2027, an upper limit of 1,577 AL tax points per doctor per billing working day is planned, measured as an average over a calendar month (General Interpretation GI-02, TARDOC catalogue 1.5). The tariff partners submitted the version for approval on 2 June 2026; the procedure is ongoing. The rule has been requested but not yet approved. This survey records how many AL tax points your practice actually generates per working day.

Confidentiality

No patient data, no names, no ZSR number. Your e-mail address (field 0.7) stays with the data acceptance office, is separated before the analysis and deleted no later than 90 days after the confirmed transfer; unaggregated raw rows are deleted on 30 June 2027. No group with fewer than five units is reported. Details are set out in the privacy notice SOG-AL-DSE-1.0.

Practice and staff

SOG-AL-DS-1.9-C · 02_Struktur · A1–A11
Structure of the practice or site
A1 required
Note on this question

You choose this code freely, for example P1. It is not the software provider's identifier from 0.1a and not the participation token from 0.1b.

A2 required
A3 Form of organisation (ownership) required
A5 required
people
Note on this question

Whole number of people (heads), regardless of employment level. People are counted, not GLNs — the number of GLNs is asked separately in 0.5.

A7 required
A4 Operating form: number of doctors and sites required
Note on this question

Independent of A3. A single-doctor practice can be investor-owned. One submission covers one practice: all doctors and all sites of this practice belong in the same submission.

A6 required
FTE
Note on this question

Decimal number in full-time equivalents, not in percent: 1.0 is one full-time post, 0.5 a half-time post. Example: two full-time posts and one 60-percent post make 2.6. Only clinical work with patients counts, without administration, research and teaching.

A8 important
h / week
A9 important
FTE
Note on this question

Decimal number in full-time equivalents as in A6, not in percent.

A10 optional
FTE
Note on this question

Decimal number in full-time equivalents as in A6; a subset of A9.

A11 important
Note on this question

Examination places in simultaneous use with their own staff. Include places where non-medical staff (MPA, TPA, optometry) carry out delegated preliminary examinations without a doctor being permanently present. Do not count waiting or refraction places without their own staff.

Equipment and software

SOG-AL-DS-1.9-C · 02_Struktur · A12–A20
Structure of the practice or site
A12–15Which equipment is available on site?required
A12OCT available on site
A13Perimeter available on site
A14Wide-field fundus camera available on site
A15Optical biometry available on site
A16 Surgical activity required
Note on this question

If you operate externally and the billing does not run through your system, surgery days appear as low-output days and distort every average.

A17 Recognised postgraduate training site — category required
Note on this question

Category of recognition as a postgraduate training site: A, B or C. If there is no recognition, choose «no recognition». If you do not know the category, choose «category not known».

A18 required
A19 Is a structured data export from this software possible in principle? required
A20 TARDOC version used for billing during the period required
Note on this question

The TARDOC version is usually shown in your practice software under the tariff settings or on the invoice. If you do not know it, choose «not known»; your participation remains valid.

Period and billing

SOG-AL-DS-1.9-C · 02_Struktur · A21–A22, B1–B4
Structure of the practice or site
A21–22Actual start of the period analysed / Actual end of the period analysedrequired
Note on this question (A21)

If your data come from your software provider, enter the period it actually delivered; if you have not been told, keep 2026-01-01.

Note on this question (A22)

An end up to 2026-07-31 is allowed if your analysis includes July. If you have not been told the period your software provider delivered, keep 2026-06-30.

Billing basis and delegation
B1 Under which GLN are services invoiced? required
B2 Under which identifier do residents (assistant doctors) bill? required
B3 During the period, do several people bill under the same GLN? required
Note on this question

This determines whether a measured excess reflects a real workload or a billing artefact.

This is the central question of the whole survey: in TARDOC, technical services are valued in doctor minutes but are mostly performed by delegated staff.
B4 What share of technical examinations is performed by non-medical staff? required
Note on this question

This is the central question of the whole survey: in TARDOC, technical services are valued in doctor minutes but are mostly performed by delegated staff.
